IFSP - Create New - Options to completing the form

  • These are the general guidelines for completing the SRS forms.  
  • You will need to complete the data as per your district manager and/or ESU director instructions.

  • Complete all of the required validation blocks as needed on the individual tabs.

  • Once completed by all users, then you can finalize.

  • If you need to make changes after finalizing, you will need to discuss with your district manager whether or not the file can be unfinalized.


Several of the forms have options that you must select prior to creating them.  

There are 4 types:

  • Annual 

    • static 364 from previous annual 

    • must meet annually

  • Initial 

    • static 364 from IFSP start 

    • periodic does not reset

  • Interim 

    • no expiration

    • not longer than 6 months

    • stop gap for up to 45 days for new needs student

  • Periodic 

    • 6 months from current IFSP

    • due date override if annual is prior to 6 month expiration


There are 4 data options:

  • Blank - enter data to complete tabs

  • Duplicate - edit data as needed on the tabs

  • Update/Modification - edit data as needed on the tabs

If you select Duplicate or Update/Modification, it will give you a list of all of the IFSPs for the student.  Choose the appropriate radio button to continue.


If any of the tab numbers are red or there is a red Validation messages bar, you will need to add data to all of those sections before you can finalize.
