504 Notes Page

The 504 Notes page is a page that captures anything you want to document that isn’t a form in SRS.  

Examples:  

• The minutes from the 504 meeting.  

• A phone call with a physician that you’d like to be documented as part of the record.

• A behavior plan for the student.

You will need to Title the form, date it and then type in the text box. This form does allow for uploads so you can attach files.
